An initial letter of inquiry, no longer than two pages including a description of the organization, the project, goals and objectives, other potential funders and a preliminary budget is required before formal application is considered. If the project meets the Foundation's criteria, a site visit will be scheduled following which the organization may be invited to submit a formal proposal. • Profile of Organization (Brief history of organization and description of programs and activities; mission statement.)
• Statement of Need
• Program Description
• Goals and Objectives (Include timeline)
• Key Staff (Description of roles; staff and FTE's. Please include paraprofessionals and volunteers, if applicable.)
• Evaluation Plan
• Replication Plan (if applicable)
• Collaborative Activities (Please identify organizations with whom you are partnering to accomplish your goals.)
• Plans for Future Funding
• Budget (Please include current funders and outstanding requests to other grant makers.)
• Annual report and audited financial statement
• Copy of 501(c)(3) authorization letter
• Organizational brochures and newsletters, if available.

Grants are reviewed three times a year, September, December and April. The deadline for each cycle is August 15, November 15, March 15.
